傷口嘅情況可以隨時間改變。觀察唔單止係睇傷口表面,亦要留意周圍皮膚、分泌物、氣味、痛楚,以及患者整體精神和身體狀況。

照顧者應留意的 5 個變化

以下任何變化都值得記錄及持續觀察;如果情況明顯、持續或同時出現多個徵狀,應尋求專業評估。

紅、腫、熱範圍增加

傷口周圍愈來愈紅、腫脹加劇,或者摸落明顯較熱,尤其當範圍持續擴大時,更值得提高警覺。

滲液/分泌物明顯改變

例如分泌物突然增多、顏色或性狀明顯改變、變得混濁,或者出現膿性分泌物。

出現異常氣味

傷口出現新嘅異味,或者氣味愈來愈明顯,特別係同時伴隨分泌物增加、紅腫等其他變化。

疼痛突然加劇

比之前明顯更痛、觸痛增加,或者原本相對穩定嘅痛楚突然出現變化,都不應只當作正常現象忽略。

發燒/全身不適

如果傷口情況變差,同時出現發燒、發冷、明顯疲倦或全身不適,應盡快尋求醫療評估。

一個警號,不代表一定感染

以上任何一個徵狀單獨出現,都唔代表一定係傷口感染。傷口變化可以由好多唔同因素造成,單靠一張相片或者一個症狀,未必能夠判斷真正原因。專業評估會綜合傷口、患者整體狀況及其他臨床資料作判斷。

哪些人士尤其需要定期觀察?

對以下人士而言,傷口變化可能較難處理,定期觀察尤其重要:

  • 長者
  • 糖尿病患者
  • 行動不便或長時間臥床人士
  • 出院後仍需要持續傷口護理的患者

幾時應該尋求醫療評估?

如果傷口變化令人擔心、持續惡化,或同時出現發燒、發冷、明顯疲倦及全身不適,應盡快尋求合資格醫護人員評估。若患者情況嚴重或急速惡化,請立即致電 999 或尋求緊急醫療協助。
